Toilet: Ek Prem Katha is a comedy-drama movie that stresses on the need for sanitation hygiene in rural areas. The movie opens with Indian women going to the fields to openly defecate behind the bushes. Keshav meets Jaya and falls in love with her. They want to marry but Panditiji is of the view that Keshav's horoscope is such that he will have to marry a woman who has two thumbs in one of her hands.
Keshav makes an artificial thumb for Jaya and they get married. On her first morning, Jaya reluctantly goes to the open fields to defecate but comes back without defecating and complains saying that she wants a toilet made. Keshav tries several attempts to make her comfortable but fails. Jaya gets frustrated and leaves the house.
Keshav tries to convince the sarpanch and villagers to build toilets in the village, but they don't agree. With the help of Jaya, he contacts the concerned regulatory authority to build a toilet in front of his house, which is demolished by his family and sarpanch, when Keshav is asleep. Keshav protects the toilet before it gets completely demolished.
Jaya files for a divorce due to the unavailability of the toilet. Politicians and government facilities work on constructing toilets quickly in the village, but Panditji is stubborn. One day, Panditji's mother, while going to defecate, falls on the doorstep and injures her hip. She says that she can't go to the fields and has to defecate in the toilet in front of their house. Panditji helps his mother in the toilet and understand the importance of toilets.
The judge gets an official notice from Cheif Minister urging the judge not to grant a divorce as the construction of toilets shall start from the next day. Panditji apologizes Jaya for his stubbornness. In the end, it is shown that toilets are being built in the village.
