Young George, who is studying for pre-degree, is infatuated with Mary, the prettiest girl in his area who has an envious fan following. Along with his friends Shambu and Koya, he follows Mary around in hopes of befriending her and makes many attempts to propose her in vain. When he finally strikes up a friendship with Mary, George comes to know to his disappointment that Mary is in a relationship with another guy who is also named George. He is left heartbroken and the poor performance in the year-end examination makes George and his friends introspect their priorities and set it right.
5 years later, George, Koya, and Shambu make it into college together. While in college, George falls for a new lecturer Malar Arivazhagan who happens to be a Tamilian. Both instantly strike up a good relationship and Malar manages to make some good influences on George. Convinced Malar Miss is in love with him, he contemplates on marriage and future. But life is shattered for George when Malar meets with an accident which leaves her with memory loss.
George is now in his 30's. He runs a cafe with his childhood friend Jojo. One day when they were about to close the cafe, young Celine walks in asking for a B'day cake. George instantly feels an affinity with Celine. Eventually, he learns that Celine is the young girl who used to accompany his teenage sweetheart Mary. Despite the age differences, George proposes Celine but comes to know that she is about to get married and loses hope. But due to her would-be's abusive behavior, Celine calls off the marriage and finally George gets married to her.
Premam instantly struck with the younger section of the audience and enjoyed a warm reception outside Kerala as well. The movie went on to collect over 60 Crores at the worldwide box office which, at the time, was a mostly an unsurpassable milestone in Mollywood.
